The Mental Health Involvement Project would like to invite you to take part in a short survey about your experience of Care and Treatment Planning.
Why have I been sent this survey?
All individuals who use Secondary Care Mental Health Services, which includes Community Mental Health Teams, are required to have a Care and Treatment Plan by the Welsh Government. This survey aims to explore whether you have been involved in creating and reviewing your Care and Treatment Plan.
There are no right or wrong answers. If you do not know whether you have been involved or have not been involved, we still invite you to complete the survey. It is important for us to hear the experiences of a range of people.
This survey has been sent to people who are receiving secondary care and has been sent to you by Cwm Taf University Health Board (UHB). It will form part of the Health Board’s process of auditing and evaluating its Care and Treatment Planning procedures.
Will my answers remain confidential?
Yes, we do not ask for your name or your Care Co-ordinator’s name, therefore you we will not be able to identify you from any answers you may give.
The survey will not be returned to the Health Board, but will be returned to the Mental Health Service User Involvement Project at Interlink who are acting as an independent evaluator.
What are the benefits of completing the survey?
It is unlikely that you will benefit personally from completing this survey. However your responses, along with other people’s responses, may inform future improvements and developments in the way Care and Treatment Plan’s are created and reviewed.
What are the disadvantages of completing the survey?
Although we do not anticipate that there will be any disadvantages in taking part, the survey will take approximately 5 minutes of your time.

Survey on Care and Treatment Planning
This survey is open to anybody who is receiving secondary mental health services in RCT or Merthyr Tydfil. If you are being supported by the local Community Mental Health team (e.g. a CPN, psychiatrist or social worker), we would be really grateful if you could spend five minutes completing this short survey on your experience of Care and Treatment Planning.

The results of the survey will be fed back to Cwm Taf Health Board and Welsh Government and will be used to help improve mental health services locally.
