Wellbeing Coordinators
Wellbeing Coordinators link adults to sources of non-medical support in the community to support their social, emotional and practical needs. They work closely with GP surgeries and community organisations improving the links between primary care and the third sector. They work with people to identify what matters most to them and link them to activities, groups and services in their community.
